Services Available
Medical services are available Monday-Thursday.
- Please visit our office in person to schedule an appointment.
- A $15 deposit is required for office visits when the appointment is made. (Deposits are not required for Physical Exams or Women's Health Exams). Payment can be made by cash or credit card; we do not accept American Express.
- You must complete these forms in the PyraMED student health portal before your appointment:
- Patient consent
- Patient registration
- No-show policy (missed appointments)
