- Only submit the following application once you have spoken to the Rental Coordinator to clear dates and discuss your event.
- Fill out and submit the form below
- Print out the application once it is created
- Sign the form
- Email a signed copy to the Facilities Rental Coordinator at the De Anza Campus
- The agreed upon date(s) is not secure until a deposit is received by the cashier's office
